Logan Health Rural Health Clinics in both Conrad and Cut Bank are excited to welcome two new Family Nurse Practitioners and a Pediatric Nurse Practitioner, starting December 2022.

Conrad native and Pediatric Nurse Practitioner, Kylee Bakker, will start seeing outpatient pediatric patients on Dec. 28, at the Logan Health Rural Health Clinics in Conrad and Cut Bank. Kylee graduated from Carroll College with her Bachelor of Science degree in Nursing in 2013 and attended graduate school at the University of Washington, where she received her Doctorate of Nursing Practice, Advanced Practice Nurse Practitioner degree and Pediatric Primary Care Nurse Practitioner certification.

After completing an Advanced Practice Provider Education Abroad in Naivasha, Kenya, Kylee further extended her pediatric education by completing an additional Fellowship at Seattle Children’s Hospital, caring for acute, complex pediatric patients from many specialties.

Since fellowship, Kylee practiced full time as a General Surgery Advanced Practice Provider at Seattle Children’s Hospital from 2019 to 2021, and most recently has been providing outpatient Pediatric Primary Care to patients at Great Falls Clinic. Kylee is excited to return home to familiar faces. “I’m most looking forward to returning to work in the community I was raised in, caring for the next generation of small town Montanans and raising my family in an amazing community,” Kylee said.

Family Nurse Practitioner Ashton Hofstad joined Logan Health Rural Health Clinic – Conrad on Dec. 5 and has started seeing patients.

Ashton was born and raised in Conrad, and later attended Carroll College, where she earned her bachelor’s degree in nursing, followed by receiving a masters degree in nursing at Montana State University. She also holds a board certification through the American Academy of Nurse Practitioners.

Ashton brings her experience and specialization in gastroenterology and hepatology to Logan Health – Conrad.

“I am aware of the unique challenges and needs of a rural community, and look forward to being able to provide comprehensive medical care for all ages,” said Ashton.

Claire Reynolds, FNP-C joined Logan Health Rural Health Clinic – Cut Bank as a Family Nurse Practitioner in November 2022.

Claire grew up in Grand Rapids, Mich. and graduated with her nursing degree from Hope College in Holland, Mich.

She later completed her Master of Science in Nursing from Loyola University Chicago.

Claire has over 14 years of combined experience as a nurse and a nurse practitioner in Intensive Care, Gastroenterology and Primary Care and has worked in Cut Bank since 2014. She completed training through the Institute for Functional Medicine (IFM) and officially became an IFM Certified Practitioner in December 2021.

“Working at the clinic has been wonderful. The team here is great and it’s a pleasure coming to work. I’m so grateful for the opportunity to be here. I’ve been in Cut Bank for over eight years now and the people here are just the best. What an honor to serve this community with a fabulous team,” Claire said.

Claire’s passions are to prevent illness through lifestyle modification and nutrition, and to promote health and healing in a gentle manner.
